How to Fill Out Auto Body Forms

At some point you will need to take your automobile into a car repair shop to have work done. Any body work done by a third-party auto-body shop will require an agreement form that you need to fill out. Some auto-body shops will fill out the details of the work involved and then ask you to sign the form so that you have an agreement about the work to be done. Be sure you fill out the form correctly or at least understand the form itself before signing the dotted line.

    Write in your personal information such as name, address, zip code and date you took the car in to the shop. Be sure to only fill in blanks and write your name in script, and do not sign any line yet. A signature is needed at the bottom of the form to show that you agree to the work involved and the cost that is estimated. Signing before the car has been inspected gives the auto-body shop the freedom to write in any cost they feel like including.

    Write in the details of your car in the second section of the form. This includes the make and model, year, odometer reading, car tag and vehicle identification number. The car details and identification numbers will be in the owner's manual in the glove box. The VIN number can also be found inside the driver-side door jamb.

    Hand the form back to the auto-body shop and let them examine your car. Once they have an idea of the work your car requires, they will fill in the form sections related to the work recommended and any parts you need to buy. The shop will clearly label both the item and the price, so be sure to check up on the math.

    Discuss all the things you want done on your car with the body shop, then have them write in the total parts, labor, subtotal, tax and total cost. Be sure all costs are clearly marked on the form, then sign the agreement at the bottom of the form. This signature verifies that you want the work done and agree to the total price.
