How to Fill Out Auto Body Repair Forms

Auto body technicians repair and restore the body and frame on wrecked or damaged automobiles. After the mechanic's assessment of damages to vehicles, customers receive an estimate of the cost, which includes labor and materials. In order to get a vehicle back to its original state, body shops may need to align and/or apply additional metal parts that include cutting, welding, sanding and then painting as the final touch.

    Complete the top portion of the repair form. List the customer's name, address and phone number. Fill in the transaction date (see reference for forms.)

    Input the vehicle's identification number (vin), odometer reading, make and model of the vehicle and the license plate number, in the appropriate area on the form. You will obtain this information during your initial assessment of the automobile.

    Describe the location on the vehicle that needs repairing, along with the name of the part, the part's serial number and quantity. List the method of repair and the price for each part.
